Employers who receive a "National Medical Support Notice" must add the designated children to the employee's insurance plan, if it meets criteria. Orders require children be enrolled in medical coverage.

Enroll according to policy provisions, noting any waiting periods or limitations which may apply.

Upon enrollment, insurance cards and/or copy of the description of coverage and effective dates to be sent to the custodial parent.

If the employee no longer works for you, complete and return Employer Response
If there is no insurance available, if the employee does not qualify for coverage, or if the amount of coverage exceeds withholding limitations please complete and return the "Employer Response", and "Notice to Employer" paperwork that you should include in the packet you receive.
